Purchase Terms & Conditions
Scheduling your procedure with BODY BY HEARTTM requires a non-refundable one-time payment of $1,000. We call this payment an Administrative Fee (Admin Fee).
This payment represents the time and hard work that our team members put in to make the administrative, clinical, and logistical arrangements for your procedure. This includes completion of forms and agreements, scheduling our nursing staff and your plastic surgeon, evaluation of your medical records by our surgery staff, assisting with financial arrangements for payment and lending, processing a medical clearance (only if required), and booking your chosen date for the procedure.
We are grateful and honored to be a highly sought after plastic surgery practice. Our patients come from across the United States, and as a result, our surgical calendar is both limited and highly sought after. Every available procedure date is a valuable and scarce resource—when a date is reserved, it prevents another patient from accessing that time. The $1,000 Admin Fee is therefore non-refundable. It reflects not only the significant planning and coordination efforts made by our team on your behalf, but also the commitment you make to move forward. Reserving your date protects your place in our schedule and ensures fairness for all patients. If you choose to finance your procedure, $1,000 of your financing amount will be applied to this fee.
BODY BY HEARTTM accepts all major credit and debit cards: Visa, MasterCard, Discover Card, and American Express. BODY BY HEARTTM also offers third-party financing options to those who apply for and are approved. Unless otherwise agreed to by BODY BY HEARTTM in writing, such financing arrangements must be made at least 30 days prior to your procedure and are coordinated with your Sales Consultant.
If you choose to cancel your procedure, the Admin Fee is non-refundable.
If you choose to reschedule your procedure date, please do so within 14 days of your selected procedure date. We are here to help coordinate any arrangements you require.
Acknowledgement & Agreement
I acknowledge that I have read and completely understand the terms and conditions of the Admin Fee policy. I authorize BODY BY HEARTTM to charge the credit card indicated in this financing agreement.
